Abstract
The Univeristy of Rochester Medical Center is working to notify patients after a doctor lost a flash drive containing patient information. The flash drive, which belonged to Dr. Jeffery Peters, contained the names, birth dates and diagnosis of some patients. Dr. Peters cannot recall which of his 837 patients were on the flash drive so letters have been sent to all of them. According to Teri D'Agostino, Dr. Peters and others attempted to locate the flash drive for 3 weeks before notifying medical center officials. "He's deeply concerned by that. These are his own patients, this is his practice, this is his image, and he cares about that. So we applaud him for coming forward," said D'Agostino. Jeffery Powell, the URMC CIO, said they medical center will being looking into encryption for all computers and flash drives.

Abstract
Tokyo Institute of Technology recently announced the loss of an external hard drive containing personal information. The hard drive, lost by a Tokyo Institute of Technology professor, contained academic results for 1,889 individuals including entrance exam scores and end of term exam scores. In addition, the drive contained draft questions for the engineering departments up coming entrance exam. Tokyo Institute of Technology plans to notify those affected with a letter of apology.
Chattanooga State Discovers Two Thousand Student Records Missing For One Year

Abstract
Chattanooga State officials recently announced that they determined a contractor lost student records early last year. The records, sent to United Imaging for digital conversion, contained the names, addresses, telephone numbers, Social Security numbers and even some High School transcripts on 2,000 students from 2007. According to Chattanooga State Vice President Jeff Olingy early last year United Imaging contacted Chattanooga State to report a problem with the student records. When Chattanooga arrived the files were in disarray, requiring Chattanooga staff to go through each of the records by hand to determine what was missing. Olingy stated the individuals affected have been notified and Chattanooga State is moving its scanning operations in-house.

Abstract
Harford Community College began notifying Department of Social Service clients after it discovered the loss of a flash drive containing personal information. The flash drive, lost by an employee in the WAGE Connection office, contained the names, Social Security numbers, application numbers and descriptions of special circumstances on 70 WAGE Connection participants. The loss of the drive was immediately reported to the college and to the Department of Social Services. A joint investigation is underway. According to the notification letter, Harford Community College will at minimum "be providing additional training to personnel".

Abstract
Harvard Law School is alerting clients of its Jamaica Plains center that a backup tape containing client data was lost. The tape contained 21,000 client records including 8,000 records with Social Security numbers. According to law school officials the data on the tape was protected by a password and username, but it was not encrypted. The tape was lost while a staff member was carrying six tapes to the Cambridge office. The tape is not labeled in any way that would indicate its content. Harvard Law School is implementing plans to encrypt all computer servers and backup tapes.
