Abstract
The University of Arizona recently announced the loss of a hard drive containing the personal information on former students. The drive, holding information on individuals enrolled at the university between 1997 and 2008, contained names and Social Security numbers of over 8,000 former students. The drive was moved during the summer to a new building and was discovered missing in October. According to the University of Arizona Dean of Students Dr. Carol Thompson, the files and drive were always under supervision, even during the move. Thompson called the incident troubling. In a notice to the affected individuals, the university asks that the former students watch for signs of possible identity theft.

Abstract
Messiah College is working to notify individuals after the loss of a drive containing personal information. The drive, which went missing earlier in November, contained the a “collection of personal information” dating from 1994 to 2010 on 43,000 students, prospective students, alumni, parents and guardians. According to Messiah Assistant Director of Public Relations Beth Lorow, the Upper Allen police have investigate the incident and found not signs of theft. Individuals with questions about the incident and if they are affected can contact the Messiah call center at 888-747-5399.

Abstract
The Univeristy of Rochester Medical Center is working to notify patients after a doctor lost a flash drive containing patient information. The flash drive, which belonged to Dr. Jeffery Peters, contained the names, birth dates and diagnosis of some patients. Dr. Peters cannot recall which of his 837 patients were on the flash drive so letters have been sent to all of them. According to Teri D'Agostino, Dr. Peters and others attempted to locate the flash drive for 3 weeks before notifying medical center officials. "He's deeply concerned by that. These are his own patients, this is his practice, this is his image, and he cares about that. So we applaud him for coming forward," said D'Agostino. Jeffery Powell, the URMC CIO, said they medical center will being looking into encryption for all computers and flash drives.

Abstract
New York University Langone Medical Center announced that an unencrypted USB drive containing patient information is missing. The USB drive, which went missing between May 7 and May 12, contained the names, medical record number, age, sex, procedure, attending physician, anesthesiologist, type of anesthesia, time of arrival in recovery room and time of discharge from recovery room for 2,500 patients. The loss of the USB drive was reported to the NYU Langone office of Security upon discovery and letters to affected individuals were sent out. NYU Langone has strict policies regarding safeguarding patient information, including encryption. In response to this incident, NYU Langone will continue to train staff on proper safeguards required by established policies.

Abstract
Tokyo Institute of Technology recently announced the loss of an external hard drive containing personal information. The hard drive, lost by a Tokyo Institute of Technology professor, contained academic results for 1,889 individuals including entrance exam scores and end of term exam scores. In addition, the drive contained draft questions for the engineering departments up coming entrance exam. Tokyo Institute of Technology plans to notify those affected with a letter of apology.
