Abstract
Long Island University is alerting students after a defect in 1098-T forms potentially exposed student information. The 1098-T Tuition Statements for 25,000 to 30,000 LIU students were mailed out in defective envelopes that were missing adhesive on one side. This missing adhesive caused the 1098-T forms to easily fall out of the envelope which cause half of the forms to be damaged during processing at the post office. While the university believes the risk of ID Theft to be low, LIU did create a web site - www.liu.edu/dataexposure.html - to help answer students questions and offer ways to protect against fraud.
